You have the renaming settings set to rename the existing file, you should be renaming the new file instead.

If you where to use the rename existing file settings, you would need to look up the record for the old file and update that as well as insert the new file.

Use the Rename New File option and the new file name will be inserted to the database and the record for the existing file will not need to be updated.
