Form throwing error on submit

So, Dreamweaver places files at the root of your site definition.

in the ftp site, the path for the email file is:
/htdocs/thilldesign/dingbatstore/InfoRequest.php

in the dingbatstore/InfoRequest.php file, the path it uses to look for thewebassist/email/waue_InfoRequest_1.php file is

include("../webassist/email/waue_InfoRequest_1.php");

meaning it is looking for the webassist folder a directory down form the dingbatstore folder at:
/htdocs/thilldesign/webassist/email/waue_InfoRequest_1.php

in the templates folder at that location there is no Center.php file


there is one at the
/htdocs/thilldesign/dingbatstore/webassist/email/

location though.

i moved the template file that was at:
/htdocs/thilldesign/dingbatstore/webassist/email/templates/Center.php

to the location it was being looked for:
/htdocs/thilldesign/webassist/email/templates/Center.php
