Errors with form submissions

I updated the page on the prowater site to use the latest version of the extension.

I also noticed that the FROM address was set to the user input field. It is better to use that as the Reply To address and send it FROM the same domain to improve deliver-ability, so I made that change as well. Please try again and let me know if that fixes it and you can copy and paste to the other site to have the same effect.

You may want to update to the latest version of DataBridge so your extensions use the latest versions of the server behaviors available soon.
