"initializeUI" and "uiClose" not defined

This sounds like a dreamweaver cache issue.

1) Close Dreamweaver
2) go to %appdata%/adobe/dreamweaver version/en_us/Configuration
3) delete the file named winfilecache_[random number].dat

Then open dreamweaver and try to use the extension. This is almost always the solution when you see the error:
"initializeUI is not defined"
