error in jquery-blockui-formprocessing.js - Cannot read property 'h5Validate' of null

I recently updated a working page and now it is getting stuck on form submission.

When I click the insert button, the progress spinner is activated and the page seems to hang.

Viewing the console, I am getting the error in jquery-blockui-formprocessing.js

Cannot read property 'h5Validate' of null
