'reply to' help please

Hello! I hope you're having a good day.

This is the first time I'm trying to add a 'reply to' to the email sent on a form submission.
The form is on this page: https://mvplodge.com/contact/

I followed the steps in this: http://www.webassist.com/help/Email/universal_email_from.html and I looked through the other forum answers to see if I could find some tips.

I tried to use the email address from the form submission, but I must have done something wrong. Dreamweaver warned me that there was a syntax error but I uploaded the page anyway and of course it wouldn't load.

The page currently doesn't have the 'reply to' code in it because I needed the form to be functional so there isn't anything for you to troubleshoot. I'm hoping you can tell me what I did wrong so I can do it correctly. Basically, all I did was open that 'reply to' panel by clicking on the 'to' and then I used the lightening to add the form email address to the 'reply to' field. I didn't add anything to any of the other fields.
