How do I prevent people from ordering outside the US if the client has paypal pro?

Also, which pages are paypal express pages vs. regular credit card checkout? Are the PP prefixes part of the express checkout as in : pp_checkout.php ?

Yes... anything pp_ is part of express checkout.

You can probably add validation to the country list on the checkout page to restrict anyone but US from moving on.
