WA_Memento.htm

I had initially installed Form Toolkit. Unfortunately, the product did not work as hoped for me and I uninstalled it. Now a dialog keeps popping up that it cannot find WA_Memento.htm whenever I start up Dreamweaver. There doesn't seem to be a way to remove that irritating dialog box.

I'm using a Mac, Dreamweaver CS5.
