Suddenly Email form give blank page

Form compile blank page. When i use Forms insert new form i made some presets. How come that this is not working anymore?
I tried to install my extension (super suite) again but with same problem

I have attached the file. On the web it is only a blank page

a blank page means that a php error is occurring but error reporting is turned off.

to turn error reporting on, add the following code at line 1:

once we know what the error is, we will be able to troubleshoot the cause.

I removed everything (webassist and spry assets..)
didnt use my form template but webassist simple contact form
and it works.

Still the spryassets pops up now and then with messages even anything has been typed.

I am not able to reproduce a problem with the spry error messages on the page you linked to.

The spry error messages appear to working correctly.

Strange. No errors in php script. And i have testet the mail to, it is ok.
Can it be the submit button.

Double check your spam folder, the email may be trapped as spam.

also, Who is your host?

depending on your host, you may need to ensure that the From Address you use is a valid email address for your domain, and you may need to check the option to include the from address as an extra parameter.

depending on your host, you may also need to enter an SMTP Server name and port in the SMTP server settings for Universal Email.
