Form throwing error on submit

The attached form throws error: Fatal error: require() [function.require]: Failed opening required 'Center.php' (include_path='.:/usr/share/php:/usr/services/vux/lib/php') in /data/20/1/125/115/1940604/user/2116827/htdocs/thilldesign/webassist/email/mailformatting_php.php on line 15
when I submit the form

Can you point me in the right direction to resolve? Thank you.

make sure to upload the entire webassist/email folder including the webassist/email/templates/Center.php file.

yes, the entire folder with Center.php is on the remote server. I found the file Center.php in waue_InfoRequest_1.php and changed the path from: $MailBody = $MailBody . (GetFromPage("../templates/Center.php"));
to
$MailBody = $MailBody . (GetFromPage("http://thilldesign.com/dingbatstore/webassist/email/templates/Center.php")); on line 40
did not work

So, Dreamweaver places files at the root of your site definition.

in the ftp site, the path for the email file is:
/htdocs/thilldesign/dingbatstore/InfoRequest.php

in the dingbatstore/InfoRequest.php file, the path it uses to look for thewebassist/email/waue_InfoRequest_1.php file is

include("../webassist/email/waue_InfoRequest_1.php");

meaning it is looking for the webassist folder a directory down form the dingbatstore folder at:
/htdocs/thilldesign/webassist/email/waue_InfoRequest_1.php

in the templates folder at that location there is no Center.php file


there is one at the
/htdocs/thilldesign/dingbatstore/webassist/email/

location though.

i moved the template file that was at:
/htdocs/thilldesign/dingbatstore/webassist/email/templates/Center.php

to the location it was being looked for:
/htdocs/thilldesign/webassist/email/templates/Center.php

Thank you, Jason, works fine now and I even understand the logic behind your explanation. Progress! Eventually will move this client back to their own server at 1and1 where we have to upgrade to a hosting package that will allow php files to connect to database.

So, I upgraded my basic 1and1 package and can now connect php files to the server. Unfortunately this file, http://dingbatstore.com/InfoRequest.php, is not connecting.

However, this file http://dingbatstore.com/InfoRequest.php5, does connect. Go figure. It throws a preformatted 1and1 error:
Error 404: NOT FOUND!
This domain name has just been registered.

I've included files in attachment and link to the site where you can see the problem occur and ftp login details in a PM so you can access the site files directly to troubleshoot.

looks to me like you have a server setting that is rewriting the page extension from:

.php

to:
.php5


you should contact 1&1 support about this problem, it looks to be a server setting issue.
