Form with Captcha has stopped submitting

I received a message from our client saying that his form stopped submitting. I sent a couple of tests through and he is stating that he is not getting them. I will also check to see if anything on the Godaddy side has changed but wanted to see if there are any issues with the programming side of things Any guidance is always appreciated. I've attached the related files

Try adding this just before the html code doctype tag:

Sounds great, I'll try that.

Side note, this seems to be happening to a couple other clients of ours. One, just stopped submitting completely. I'm contacting GoDaddy on that one to see what's happening on the server side of things but very baffling that this is happening seemingly out of no where.

Maybe they are updating php version on you. That can cause issues with older code.
