Ongoing Work

I was talking to Ray last year when he was sick and so sad to hear of his passing. He used to help me with little things and I now don't know who to turn to. I have a web site I maintain for a company that I designed with Dreamweaver and using Webassist MySQLi and a lot of Ray's stuff. I have simple questions from time to time and am wondering if anyone would like to pick up where Ray left off helping me out, for pay of course.

Sorry Vernon that I wasn't able to spot the issue. But I am guessing others here will be able to help.

Scope: This is a WA form that Ray customized to do a show/hide feature depending on what selection was chosen.
ie, if Yes is the answer, then a pull down for choices, and if No was the answer, a different set of choices. And answers need to be "required."

I don't use WA forms, so I wasn't helpful.
