Payment provider problem

I have had complaints from customers they cannot conclude the payment through Paypal. They get to the 'confirm it is you' page and then they get redirected back to my 'cart.php' page saying the cart is empty and the transaction has not gone through. Whatever can be the problem? I would really appreciate if someone can help me

I only have one eCart site that uses Paypal and have just done a test purchase but I don't get a "confirm it is you" option anywhere. Is this appearing in the Paypal window? What version of PHP are you using and are all your files up to date?
