Ok. Thanks. I also created a version of the same page but, with 2 drop-down menus that allow multiple selections to see if the other form field types were the issue but, I still had the same results. That url is https://sdpc.a4l.org/add_data_elements_multiple_state2.php?state=MA&softwareID=409
